YOUR FIRST VISIT

  Here is an easy way to start your therapy experience with me.  Download the forms below and bring in your information at the first session, or you can email them to me at Karen@karenrippy.com.  Everyone attending should complete and sign the paperwork.  If you are involving children in our sessions please be sure both divorced parents agree to counseling if you share legal custody.  I can give you an easy form to complete during our visit.

If you plan on submitting my billing form to your insurance bring in your insurance card.  We keep a credit/debit card on file for you.  You may keep a check on file if you prefer. 

I like to make your first visit a comfortable and productive one.  If you have specific questions please call me or email me before our visit.
